PSS Home from Hospital helps people get out of hospital sooner and safely
The PSS Shared Lives Home from Hospital scheme provides up to six weeks of intermediate care for adults with support needs leaving hospital, helping them to make the transition from hospital to an independent (or supported) home life in Liverpool, Manchester, the Wirral, the Midlands, Wales, Norfolk, Suffolk and Lincolnshire.
High quality care to get people back on their feet
Our professional carers welcome individuals into their homes to stay with them as part of our Shared Lives service. Our specially-trained carers will help individuals to live the life they would normally lead at home, by providing them with as much or as little help as they want or need. The aim is to increase the person’s confidence and practical abilities so that they can live independently (or, if necessary, with help from a carer) in a non-clinical environment.
Our Home from Hospital Shared Lives carers can provide the people they support with:
- help with practical, everyday things like washing, dressing, eating and taking medication;
- emotional support;
- an understanding of the condition they are suffering from, helping them learn how to manage it correctly;
- support to maintain contact with their friends, family and community;
- a comprehensive social care assessment, to ensure each person has the right level of support in place for their lives ahead; and
- staying in touch with the hospital, to make sure they know how and when to contact the hospital discharge coordinator and any multiagency teams
What are the benefits of the PSS Home from Hospital service?

- We’ll understand, assess and plan for the person we’re supporting’s ongoing social care needs.
- Support individuals to find their independence again and reduce the need to seek unwanted residential care
- Shared Lives helps meet NHS preventative aims.
- We’ll ensure person knows how to manage their condition, reducing likelihood of readmission.
- Reduce social isolation.
- We provide a follow-up service: someone from your local PSS Shared Lives team will visit the person being supported six weeks after their placement ends to assess if any further support is needed
How to make a referral
We accept referrals from social care professionals, NHS teams, families and friends of adults with support needs and adults with support needs themselves.
